The CRM Account Provisioning form needs to be completed by the supervisor. Please note that ITS will provide basic application training (videos and written documentation) on CRM usage, but the supervisor/department is responsible for training the employee in completing business processes correctly. Consequently, a test account will be provisioned first. After training has been completed, please respond to the ticket with a request to also provision the production account.
