Completing Your Student Agreement & Information Form (SAIF)
The Student Agreement & Information Form (SAIF) makes sure Tulsa Tech has the most up-to-date information about you. All full-time students are required to complete this form online.
You are also encouraged to submit the SAIF anytime during the school year if your information changes (for example: new phone number, address, or emergency contact updates). This helps keep your records accurate and ensures Tulsa Tech can contact you or your family when needed.
Requirements for Completing the SAIF Form

Enrollment must be active: Your enrollment must show as current for the form to open.
How to Access the Form
- Go to login.tulsatech.edu.
- On your dashboard, click the "Complete the SAIF" link.

What You’ll See in the Form
- Personal & Contact Information
- If you change your name, date of birth, gender, or Social Security Number or Tax Identification Number, documentation is required. You may be asked to provide documentation by campus staff if you choose not to upload documentation at the time of the change. These changes will be reviewed by campus staff before they update in our system.
- At least one phone number (Home or Cell) is required.
- Additional Information
- If you are a Freshman – Senior, you will see your Current High School and Current Grade Level. Changes to your Current High School or Current Grade Level will be reviewed by staff before updating in the system.
- If you are an Adult student, you will see your Current Grade Level and Education Level. Changes to your Current Grade Level will be reviewed by staff before updating in the system.
- If you select American Indian/Alaska Native, you can choose up to five tribal affiliations from a searchable list. Start typing part of your tribe’s name in the box to quickly find and select it.
- Parent/Guardian Information
- This section appears if you are a Freshman – Senior. At least one Parent/Guardian is required. Changes to Parent/Guardian information will be reviewed by staff before updating in the system.
- This section is not shown if you are an Adult student.
- You will need to enter First Name, Last Name, Primary Phone, Email Address, Relationship to you, and whether you live with this person.
- Emergency Contact Information
- At least one Emergency Contact is required if you are an Adult student.
- Emergency Contacts are optional if you are a Freshman – Senior. If you add one, all fields must be filled in for it to save. Do not re-enter Parent/Guardian information here because they are already counted as Emergency Contacts.
- You will need to enter First Name, Last Name, Relationship to you, and a Primary Phone number.
- Medical Information
- You can list any medications you take. Please add an additional line for each medication.
- You can check health conditions such as allergies, asthma, diabetes, or seizures.
- You can also provide additional Health Information details, and list a Preferred Physician or Preferred Hospital if you wish.
- Auto Information
- You can add your vehicle information.
- If you enter a Driver’s License State, you must also enter your Driver’s License Number.
- If you enter a Driver’s License Number, you must also select a Driver’s License State.
- E-Rate Program
- You must answer Yes or No.
- Your response helps Tulsa Tech qualify for federal technology funding.
- State Requested Statistical Information
- You do not have to answer these questions to submit the form.
- Consent & Agreements
- High School students will see a Field Trip Permission question.
- All students will see a Talent Release Permission question.
- Acknowledgement
- You must digitally sign the form to confirm your information is correct.
- Submitting the Form
- After all required fields are complete and you have signed, you may submit the form.
- You will see a confirmation message on screen.
When Will My Changes Show?
GOOD TO KNOW:
Most updates are automatically sent to our system. Any updates to your legal name, date of birth, gender, high school, SSN/TIN, grade level, or Parent/Guardian information must be reviewed and approved by a Tulsa Tech staff member.
Parent/Guardian Signature for High School Students
If you are a High School student, after you complete and submit the SAIF, the system will automatically send an email to your parent/guardian(s). They must digitally sign your SAIF to finish the process.
Frequently Asked Questions
Q: Who has to complete the SAIF?
A: All current full-time students must complete the SAIF each school year. It is recommended that students complete a new SAIF anytime important information changes.
Q: Do I have to answer the "State Requested Statistical Information" questions?
A: No. That section is always visible, but all questions are optional. You can submit the SAIF without answering them.
Q: Why does my parent/guardian receive an email to sign the form?
A: If you are a High School student, your parent/guardian will receive an email after you submit the SAIF. They must digitally sign the form to complete your submission.
Q: I updated my information. When will I see the changes?
A: Most updates process within a short delay. However, any updates to your legal name, date of birth, gender, high school, SSN/TIN, grade level, or Parent/Guardian information must be reviewed and approved by a Tulsa Tech staff member.
